Maximum Temporary Income Benefits:        $1,112.00
Minimum Temporary Income Benefits:         $   167.00
Maximum Impairment Income Benefits:       $   778.00
Minimum Impairment Income Benefits:        $   167.00
Maximum Supplemental Income Benefits:   $   778.00
Maximum Lifetime Income Benefits:            $1,112.00
